The Impact of the Chronic Care Model on Health Outcomes and Patient Satisfaction Among Elderly Patients: A Scoping Review

Abstract

Background: Chronic diseases significantly impact the elderly, who require comprehensive care to minimize complications. The Chronic Care Model (CCM) offers a systematic approach to restructuring medical care, enhancing decision support, promoting self-management, improving clinical information for health systems, and providing better access to community resources.

 

Objective: This study aims to explore how chronic care management improves health outcomes for elderly patients.

 

Methodology: A scoping review was conducted to address two primary questions: “How does the Chronic Care Model (CCM) lead to satisfaction among elderly individuals with chronic conditions?” and “What roles do health providers play in the Chronic Care Model (CCM) to achieve such satisfaction?” Various databases were searched for relevant literature. Content thematic analysis was employed to gain a deeper understanding of predetermined themes, including the chronic care models, principles, and programs, their implementation, the role of nurses in executing and achieving the outcomes of these models, and patient outcomes such as satisfaction and experience.

 

Results: The review included 25 articles from various settings, regions, and countries. The findings confirmed the positive role of adopting the Chronic Care Model in enhancing patient experience and satisfaction in chronic care settings. Additionally, the study highlighted the crucial role of nurses in achieving desired patient outcomes, including clinical outcomes, high-quality care, and patient satisfaction.

 

Conclusion: Adopting the Chronic Care Model significantly enhances the satisfaction and experience of elderly adults. Further research is recommended to explore the adoption of CCM and its impact in developing countries.
